This resource includes three templates in both print-and-go and editable formats: disciplinary absence, medical absence, and other absence.
Simply fill in the blanks to capture logistical information, the student’s perspective, how to handle missed work, social-emotional supports, and action steps for the student, school, and family. The disciplinary re-entry plan also includes restorative responses and the medical re-entry plan lists symptoms, side effects, and any school-based medical support. This form will guide discussion at a re-entry conference and can be given to the family and any school-based implementors.
There is also a re-entry reflection for the student that will guide them through identifying their coping skills and provide invaluable information about how the school can best help them transition back.
