Behavioral data is a key tool for crafting effective behavior interventions. Unfortunately, those who have the most opportunities to observe behaviors are often overburdened with other tasks and struggle to document it consistently. This form is designed to be as easy and quick to use as possible so that as much data as possible is collected.

 

This data collection form uses antecedent-behavior-consequence (ABC) analysis to gather information that will help you determine the function of the behavior and thus choose an appropriate replacement. Simply supply copies to anyone who has occasion to observe the student's behavior and ask them to fill it out either whenever a challenging behavior arises or only when a specific target behavior arises. There is a space for added notes if they have anything to add. Collect the forms (ideally representing a variety of settings) and use them to inform your behavior intervention.
