The requirements for continuing education units towards licensure are complicated, hard to remember, and always changing. This spreadsheet will keep you organized so that you don’t fall behind and so that you don’t have to stress trying to dig through old CEU certificates to make sure you met all of the requirements.
 
Next time you attend a professional development session that counts towards license renewal, simply fill out the log. First, you will enter the date you attended, the name of the training, and the number of hours that you received credit for. Using the drop down menus, you will indicate whether those credits count towards any of the specific requirements for licensure and whether the credits were official CEUs or not.
 
The form will then autocalculate for you and let you know how close you are to meeting the overall hour requirements and the requirements for particular content areas. You will be able to track your progress at a glance and the hours will automatically turn green when you meet a requirement in full. Right now, this form is designed for Illinois and Michigan LCSW credit hours. However, if you are in another state or have another
